My friend is diagnosed as 'psychotic' from the original diagnosis of 'bipolar'. He really needs to see a...highly qualified specialist...or at least someone more knowledgeable in the field of mental health than a nurse practioner. The problem is that he does not have very good health insurance and currently does not work. He did inherit a good bit of money...low six figures...but it is not enough to sustain him his entire life...nor is it enough to pay individually for these visits. His medications need to be fine-tuned for him and he needs a clear diagnosis. He cannot qualify for good health insurance (like a Blue Cross PPO) because of his poor health and obese status. What should he do? Getting a job with benefits would be quite a stretch for him at this point. Thank you in advance for your answers and suggestions.

Well, people who are bipolar can become psychotic, esp. when manic, so don't totally discount it, BUT you're right, he def. needs to see someone besides a nurse practitioner.

IMO, if he uses some of his money for good treatment, he'll be able to function and then earn the money he needs to support himself with a job that has benefits. But he has to get a good doctor.

That might really be the only thing to do. I'd say treatment first, using some of his inheritance, and then look for a job, etc. He really has to research possible docs to be sure he has a good one, though. There are some crappy ones out there. Also, he needs to make sure he doesn't get stuck with meds that contribute to weight gain, since he already has that issue.

Bipolar individuals can go into and out of "psychotic" states without necessarily requiring a permanent change in their diagnosis. And, you're right, a nurse-practitioner is not qualified to properly diagnose such a complex case. Unfortunately, due to his inheritance wealth, he will not qualify for low fees at a Community Mental Health Center, even if it has a sliding scale fee arrangement. His best bet, I think, is to ask to see a psychiatrist at such a mental health center with the explicit and limited purpose of medication management. He may be able, after a few initial diagnostic sessions, to get away with quarterly visits once he's on an effective medication regimen. The charges to see a psychiatrist at a Community Mental Health Center will probably be lower than to see a private psychiatrist.

Does he qualify for state aid? Check into the local organizations and see if they can help, places such as local churches, etc. I am sure there is some sort of aid he will qualify for. Perhaps his local health dept can direct him in the right place. Best of luck to you both.
