A psychiatrist's case files would generally be considered a primary source, particular in they had important patients.
Or if the psychiatrist was important in their own right, say Sigmund Freud, his files would be considered primary.
One of the best ways to judge whether a source is primary or secondary is to ask: Was it contemporaneous? Did it happen at the same time as the event you are studying, or is it an explanation written afterwards. Caveat: newspaper accounts would often be considered primary sources.

Primary...however if you were aksed to interview the psychiatrist that would be a secondary source.
