Is it ethical for a mental health organization to alter a clients interaction with society by creating?

Is it ethical for a mental health organization to alter a clients interaction with society by creating?therapeutic situations (without the clients knowledge) outside the office?..for instance Them asking a person whom causes distress to the client to be at a certain place where the client is know to attend regularly..(for exposure therapy)?

I don't know. Is it ethical for the CDC to improve people's immune systems by creating theraputic situations (without the clients knowledge) in the outside world? For instance, placing a certain virus that they think the clients need a better immune system for... at a certain place where the client is known to attend regularly (for exposure therapy)?

no, that's fucked up. it makes the client question everyone around them wondering if everyone is a fake.
