How about having them write up their own support system for when they leave the hospital. This would include support groups, friends and family who are supportive, doctors, etc. Have them make up a fire drill. A fire drill is what they would do if they felt mental or like using drugs or alcohol. I use it with my clients. Here's an example.

If I feel uneasy or unsafe I will'
1. Call my doctor
2. Go to the ER
3. Call my parents
4. Call my support group
5. Go to a meeting
You would post the numbers, times, and places
I work in addictions but my clients also have a dual diagnosis. Having a written plan for support for when they get out is good and having a fire drill for the in case of emergency times is good too.
