Can i request a copy of my file from mental health?

Can i request a copy of my file from mental health?I was wondering if I am allowed to have a copy of my file including all of my therapists notes. And is there a form to fill out? It's a NY State county mental health building. Thanks!

No, you can't do that in any state. Only a physician can.

I don't know about the county health building, but I was able to get my progress notes from my psychiatrist. When I filled out the request form there was a little note that said you cannot get therapist notes. I don't know this was in Minnestoa so I'm not sure about your state.
